Il Lombardia Under 23 is a UCI 1.2U one-day road race for men under 23 through the hilly terrain of Lombardy, combining long paved roads, rolling sections, steep climbs and technical descents. The course typically covers around 160–180 km and features substantial elevation gain, with several climbs ranging from sustained 5–8% gradients to sharper ramps above 10%. Narrow roads, wooded sections and frequent changes of direction demand precise positioning, particularly before the climbs and on fast downhill sections. The race is defined by repeated elevation changes rather than a single uninterrupted ascent, requiring riders to balance aggressive efforts with careful energy management. Early breakaways can form across the rolling terrain, while the peloton gradually becomes smaller as the major climbs accumulate. Groups may regroup on descents or flatter valley roads before the next ascent reshapes the race. In the later stages, attacks and counterattacks become increasingly important, with the strongest riders seeking separation on the final climbs. The finish can come from a solo rider, small breakaway or reduced-group sprint.
